§ 33.12 PREREQUISITE QUALIFICATIONS AND REQUIREMENTS OF APPLICANTS FOR FIREFIGHTER.
   Prior to being administered any written or other examination for the position of firefighter, each applicant must meet the following prerequisite requirements. Applicant must:
   (A)   On the date of submission of the application be 21 years of age and not over 40 years of age and provide a copy of the birth certificate.
   (B)   Be a high school graduate or must have obtained a general education development (GED) diploma for high school level and provide a copy of the high school diploma or official certificate of receipt of GED;
   (C)   Be an orderly, law abiding citizen;
   (D)   Be a person of sobriety and integrity;
   (E)   Be of sound mind and body;
   (F)   Be willing to become a qualified voter of the Commonwealth of Kentucky within six months of date of appointment, if employed;
   (G)   Be willing to undergo a thorough personal background investigation prior to a conditional offer of employment, as part of the employment screening process.
   (H)   After a conditional offer of employment, successfully complete a thorough physical examination as required prior to probationary appointment provided he satisfactorily passes all other performance requirements;
   (I)   Have any other qualifications as may be prescribed by Kentucky Statutes or ordinances of the City of Ashland, Kentucky, including, but not limited to, Candidate Physical Ability Test (CPAT) per KRS 95A.040;
   (J)   As a pre-condition to employment and a condition to continued employment, execute a waiver to submit to random testing for illicit drugs. This waiver shall be in force as long as employed by the Ashland Fire Department;
   (K)   Provide a truthful statement certifying he has not used illicit drugs for the past 12 months;
   (L)   Be willing to submit to and successfully complete a polygraph test at option of the city prior to probationary appointment;
   (M)   Possess and maintain a valid driver’s license and provide a copy;
   (N)   If a discharged veteran of the armed forces of the United States) present with his application, official documentation, or DD 214, official discharge, showing reason for discharge. Applicant must have received an honorable discharge;
   (O)   Acquire and maintain Kentucky emergency medical technician certification within two years of employment.
