921.30 DISCLOSURE REQUIREMENTS.
   Each industrial user, as defined in Section 921.01, discharging or proposing to discharge to the sewage works, shall file Discharge Reports and Strength Analysis Reports with the Chief, I.P.P.
   (a)    Discharge Reports. The information on the Discharge Report (also known as a Baseline Monitoring Report (BMR)) shall be current; whenever the user's process changes, the information shall be revised. The information to be submitted shall be as follows:
      (1)    Name, address and SIC code number of user.
      (2)    Disclosure of the wastewater constituents and characteristics, including but not limited to those identified in Section 921.10 .
      (3)    Time and duration of discharges, including an explanation of all surges of strength and flow rates.
      (4)    Average daily flow rate, including method of determination.
      (5)    Site, mechanical plans and details showing size, elevation and location of all connections to public sanitary sewers, control manholes or sampling access locations and pretreatment equipment.
      (6)    Description of facilities, manufacturing processes or service activities on the premises.
      (7)    Nature and concentration of any discharge pollutants prohibited or limited by this Chapter.
      (8)    A statement detailing whether pretreatment or additional operation and maintenance procedures are required to comply with the provisions of this Chapter, (see Section 921.83 compliance schedule).
      (9)    Products manufactured or services rendered by type, amount and rate of production.
      (10)    Type and amount of raw materials used.
The completed Discharge Report signed by an authorized representative as specified in Section 921.35 , shall be sent to Ashland's Chief, I.P.P. If during evaluation additional information is required, then the user shall be requested to supply the necessary information. The Chief, I.P.P. shall notify the user in writing when the report is acceptable.
New sources, as well as those existing source dischargers which previously directly discharged wastewater to the waters of the State, that become industrial users subsequent to the promulgation of an applicable categorical standard, must submit a Baseline Monitoring Report (BMR) within ninety (90) days prior to commencement of discharge to Ashland's POTW. Estimates on production, flow and the presence of regulated pollutants will be accepted from new source discharges, and the requirement to certify compliance with categorical standards or submit a compliance schedule shall be excused. A compliance report shall be required as specified in Section 921.30 (b).
   (b)    Compliance Report. Within ninety (90) days following the date for final compliance by the Discharger with applicable Pretreatment Standards set forth in this chapter or ninety (90) days following commencement of the introduction of wastewater into the POTW by a New Source discharger, all industrial users subject to this chapter shall submit to Ashland's IPP, a report indicating: the nature and concentration of all prohibited or regulated substances contained in its discharge, and the average and maximum daily flow in gallons. If an industrial user is subject to a categorical pretreatment standard, a Compliance Report shall be submitted to the Chief, I.P.P. within ninety (90) days following the commencement of the introduction of wastewater by a new industrial user. This report shall include:
      (1)    The average discharge flow;
      (2)    The nature and concentration of any prohibited or regulated substance contained in the discharge; and,
      (3)    A statement indicating whether applicable pretreatment standards are being met on a consistent basis, and if not, the proposed measures to achieve compliance with these standards.
This report shall be signed by an authorized representative as specified in Section 921.35 .
   (c)    Periodic Compliance Reporting. Periodic compliance reports shall be submitted as required by individual industrial discharge permits. These reports shall indicate the nature and concentration of prohibited or regulated substances in the user's discharge. This report shall also include a record of all measured average and maximum flows during the reporting period, the results of sampling and analyses and other information required by the Chief, I. P. P. This report shall be signed by an authorized representative as specified in Section 921.35 .
