All reports shall be signed:
(a) "By a responsible corporate officer," (president, secretary, treasurer, or vice president) of the corporation;
(b) By a general partner or proprietor of a partnership or sole proprietorship respectively;
(c) By a duly authorized representative of the individual designated in paragraph (a) or (b) if the authorization is made in writing by the described individual to the City of Ashland's I.P.P., and the authorized individual has responsibility for overall operation of the facility from which the discharge originates or;
(d) If previous authorization is no longer accurate because a different individual or position is responsible for overall operation of the facility, in which case new authorization must be submitted to the City of Ashland's I.P.P. prior to any reports being signed by an authorized representative.
