§ 110.21  LICENSE APPLICATION.
   In addition to any other information deemed necessary by the town, the application for an “exhibitor’s license” or an “amusement machine license” shall contain the following information:
   (A)   The name and address of any person other than the applicant who holds any right, title or interest in or to each amusement device for which a license is sought; in those instances where such person is other than an individual, there shall be filed with the Clerk-Treasurer and kept available for public inspection a statement showing, in the case of a partnership, the names and addresses of the stockholders and directors and, in the case of a business trust, the names of the trustee or trustees and beneficiary or beneficiaries;
   (B)   In the case of owners, the number of amusement devices being placed for exhibition or exhibited in the town;
   (C)   In the case of exhibitors, the numbers of amusement devices and the place where the amusement devices are to be located, and whether such machines are being maintained and operated in connection with any other business or calling and, if so, the character of said business or calling;
   (D)   In case of an amusement machine license, the type of machine, its model and serial numbers, the machine’s approximate age, the name and address of the owner of the machine (the term “owner” is defined in § 110.02 of this chapter) and the applicant shall also furnish the information required by divisions (A), (B) and (C) above; and
   (E)   The name, address and telephone numbers of the operator(s) who will manage and be on duty during those hours the business is open to customers.
