If a taxicab is involved in an accident resulting in injury to person or property or an accident occurs while a taxicab is occupied by a passenger, a written report of the accident shall be submitted to the Department within three days after the accident. The report shall include the following information: date and time of the accident; operator's name and license number; vehicle license number; extent of the damage; and whether the cab was in service at the time of the accident.
