8-3-4: HANDICAPPED PARKING:
   A.   Decal Issued: The City shall issue a special decal or device to a physically handicapped person upon his application, which decal or device shall be the property of such handicapped individual and may be used by that person to designate and identify a vehicle when the vehicle is being used to transport said handicapped person, and thus is entitled to enjoy all the privileges that would be afforded a handicapped licensed vehicle under the provisions of the Illinois Motor Vehicle Code. The decal or device shall be the design, size and color of a handicapped motorist decal or device established by administrative rules of the Illinois Secretary of State, in force from time to time. The aforesaid special decal or device shall be officially referred to as a handicapped parking decal.
   B.   Application For Decal: A person desiring to obtain a handicapped parking decal, as defined in this Section, shall apply for same by written application to the office of the City Clerk upon forms to be provided by said office. Said form of application shall contain information disclosing the name and address of the applicant, the applicant date of birth, height, weight and other pertinent physical characteristics for identification, and a brief description of the physical handicap of the applicant. Said application must also be accompanied by a statement certified by a licensed physician of the State to the effect that such applicant is a person who has permanently lost the use of a leg or both legs, or an arm or both arms, or any combination thereof, or any person who is so severely disabled as to be unable to move without the aid of crutches or a wheelchair. The office of the City Clerk shall issue a handicapped parking decal to the applicant upon receipt of the properly completed application form as established by the office of the City Clerk, along with the aforesaid verified statement of a licensed physician, unless the City Clerk has reasons to suspect the validity, completeness or truthfulness of said documents, in which case the City Clerk shall so notify the applicant to submit additional data or evidence as required. No fee shall be required in connection with the application or issuance of a handicapped parking decal.
   C.   Display Of Decal: The handicapped parking decal, issued to a handicapped person pursuant to this Section, shall be used by that person to designate and identify a vehicle which is being used to transport said handicapped person, and thus is entitled to enjoy all the privileges that would be afforded a handicapped licensed vehicle, when said handicapped parking decal is displayed through the front windshield of a motor vehicle in such a manner as to be clearly visible from the outside of said motor vehicle.    D.   Restricted Parking: It shall be prohibited to park any motor vehicle which is not bearing registration plates or decals issued to a physically handicapped person pursuant to sections 3-616 or 11-1301.2 or to a disabled veteran pursuant to section 3-609, Illinois Vehicle Code, as the same is now in force and effect, as evidence that the vehicle is operated by or for a handicapped person or disabled veteran, in any parking place, including any private or public off-street parking facility specifically reserved, by the posting of an official sign, for motor vehicles bearing such restriction plates.    E.   Signs And Markings: The signs to be utilized in parking areas enumerated in subsection D of this Section shall be the same as the R7-8 sign adopted by the United States Department of Transportation made a part hereof by reference. Said signs shall also have affixed below the words "$100.00 Fine" as enumerated in appendix D 1 . (Ord. 5316, 2-8-1989; amd. Ord. 6316, 8-26-1998)
