(A) (1) A certificate of death must be secured from a licensed physician prior to the interment of any remains within a cemetery.
(2) The certificate shall give the name and residence of the deceased, the name of the nearest relative, and the time, place and cause of death. A copy of the certificate of death shall be furnished upon the request of the town.
(B) In the instance a body is brought in from out of state, no interment shall be made without a permit from the County Health Department.
