Application for such license shall be made to the Director of Public Safety upon such blank forms as shall be prescribed therefor. On such applications the applicant shall state: his or its name; whether an individual, partnership or corporation; if a partnership, the names and addresses of the partners composing the same; the addresses at which it is desired to maintain and operate the mechanical musical instrument; the nature of the business conducted at such place; the make, name, model, serial number and other identifying information with reference to the particular mechanical musical instruments which it is desired to operate; the name and address of the owner of such instruments, and such other and further information as the Director, in his discretion may require.
