729.06 FEES AND APPLICATION FOR AMUSEMENT ARCADE LICENSE.
   (a)   Every applicant for an amusement arcade license shall complete a written application prescribed by the Director of Public Safety and submit an application fee of:
      (1)   Five hundred dollars ($500.00) if the arcade will have less than eleven (11) mechanical amusement devices;
      (2)   One thousand dollars ($1,000), if the arcade will have more than ten (10) but less than twenty (20) mechanical amusement devices; or
      (3)   One thousand five hundred dollars ($1,500) if the arcade will have twenty (20) or more mechanical amusement devices.
   (b)   The application shall provide all of the following information; the name and address of the owner and exhibitor of the arcade; the address of the arcade; a copy of the conditional use permit issued by the Board of Zoning Appeals for the arcade; a copy of the occupancy permit issued by the County Code Inspector; the date of birth and social security number of the owner and exhibitor of the arcade; a list of all criminal convictions against the owner and exhibitor of the arcade; the number of machines intended to be exhibited; and an affidavit as to th good moral character of the owner and exhibitor of the arcade.
   (c)   The Public Safety Director may deny any applicant who is not of good moral character, i.e., a person who has criminal convictions for felonies or for any misdemeanors involving dishonesty or moral turpitude, or is not in compliance with any of the zoning, business, fire or buildings codes of the City of Zanesville.
   (d)   Upon approval of the application, the Director shall issue a license which shall contain the name and address of the arcade, the number of mechanical amusement devices permitted to be exhibited, and the name of the approved owner and exhibitor. A license desiring to exhibit additional mechanical amusement devices at the arcade during the license period, shall first apply for an amended license to cover the exhibition of such additional mechanical amusement devices.
