729.05 FEES AND APPLICATION FOR MECHANICAL AMUSEMENT DEVICE LICENSE.
   Every applicant for a mechanical amusement device license shall complete a written application prescribed by the Director of Public Safety and submit an application fee. The application fee shall be one hundred dollars ($100.00) for each license requested for a “Skill-based amusement machine” as defined in Chapter 2915 of the Ohio Revised Code, unless said machine only awards merchandise prizes clearly of a nature intended for minor children. The application fee shall be twenty-five dollars ($25.00) for each license requested for all other mechanical amusement devices including “Skill-based amusement machines” which award merchandise prizes clearly of a nature intended for minor children. The application shall provide all of the following information: the name of the device; the name and address of the manufacturer of the device; the model number of the device; the serial number of the device; a photograph of the device; a written description of the manner in which the device is played including the object of the game, contest or amusement; the name, address and social security number of the owner of the device and if different, the applicant. Upon approval of the application, the Director of Public Safety shall issue a decal which shall be affixed to the device in a prominent location so that all persons playing the device may see that the device has been licensed by the City of Zanesville.
