Every applicant for a license shall file with the Director of Public Safety a written application, stating the full name and address including the street and number of the applicant, or if more than one person, or if an association or firm, the full names of all parties interested and the addresses including street and number.
The application shall also state the following:
(a) The premises where such billiard parlor or poolroom is to be conducted, including street and number.
(b) The age of the applicants.
(c) Whether the applicants are citizens of the United States.
(d) Whether the applicants have ever been engaged in operating a billiard parlor or poolroom; when, where and how long in each place within the last five years.
(e) The name of the owner of the premises in which such billiard parlor or poolroom is to be located and the complete address of such owner. The application shall be signed by the applicants.
