The preliminary plat shall be accompanied by the following materials:
(A) A statement of the proposed uses of lots, stating the type of residential buildings with number of proposed dwelling units and the type of business or industry so as to reveal the effect of the development on traffic, fire hazards or congestion of population;
(B) Proposed covenants and restrictions, set out in full;
(C) A description of proposed provision for storm water drainage and sewage disposal, including the results of any percolation tests or soil borings that may be required by §§ 152.085 et seq. and including satisfactory evidence that the proposed sewage disposal methods meet the requirements of the Indiana State Board of Health;
(D) Proposed zoning plan for the area, if any zoning changes are contemplated;
(E) A centerline profile of all proposed streets or roads, with typical cross-sections indicating proposed crowns and slopes anticipated in meeting required street improvements. All drainage structures shall be indicated on such profile. All proposed grades shall be indicated in complete detail in the profiles and referenced to the stationing shown on the plan;
(F) A description of the proposed water supply system and evidence that it meets the requirements of state, county and town regulations, including such tests or logs as may be required by §§ 152.085 et seq.;
(G) Statement of the date upon which it is proposed that the development is to be commenced and completed, once it is approved;
(H) Such other information as may be deemed necessary for proper review of the preliminary plat.
