A person or entity who operates a quadricycle in the city shall submit an application with the following requirements:
(A) The applicant’s name, address, telephone number and email;
(B) A copy of a current insurance certificate providing coverage for bodily injury and property damage liability with a minimum combined single limit of $2,000,000 for all persons injured or for property damage;
(C) Proof of ownership of the quadricycle; and
(D) An executed hold harmless agreement in a form provided by the city which indemnifies and holds the city harmless from all claims arising from the operation of the quadricycle.
