Employees have certain leave rights and obligations under the Family and Medical Leave Act of 1993 (“FMLA”). The Village hereby adopts Exhibit “A” attached to Ordinance 1977-22 consisting of three pages and federal forms WH-380 and 381 “Certificate of Health Care Provider” and “Employer Responses to Employee Request of FMLA Leave”, respectively, for use in the Village.
