(a) Village residents who qualify for Village snow plowing and who desire plowing services must obtain an application therefor at the office of the Mayor.
(b) Upon completion, the application and, where appropriate, a doctor's letter indicating a disability disabling the person from performing snow removal and a signed release of the Village from liability for damage incurred during snow plow activity shall be returned by mail or in person to the office of the Mayor. The specific disability need not be disclosed to qualify.
