§ 154.236 APPLICATION FOR ZONING CERTIFICATE.
   (A)   Every application for a building permit shall contain or have affixed to it an application for a zoning certificate containing the following information:
      (1)   Nature of the proposed use.
      (2)   Number of dwelling units, if any.
      (3)   Lot area in square feet.
      (4)   Lot width in feet.
      (5)   Percentage of lot to be covered by buildings or structures.
      (6)   Square feet of floor area computed as defined in this chapter for off-street parking and loading; and for floor area ratio, if applicable.
      (7)   Size in feet of front, side, and rear yards.
      (8)   Size and location of signs.
      (9)   Number and size of required off-street parking and loading spaces.
      (10) A plat, in duplicate, of the piece or parcel of land, lot, lots, block or blocks, or parts or portions thereof, drawn to scale showing the actual dimensions of the piece or parcel, lot, lots, block or blocks, or parts or portions thereof, according to the registered or recorded plot of such land.
      (11) A plot plan, in duplicate, drawn to scale in such form as may, from time to time, be prescribed by the Zoning Administrator showing the ground area, height, and bulk of the building or structure, the building lines in relation to lot lines, the use to be made of the building or structure or land, and such other information as may be required by the Zoning Administrator for the proper enforcement of this chapter.
   (B)   One copy each of the plat and the plot plan shall be attached to the application for a building permit when it is submitted to the Office of the Zoning Administrator for a zoning certificate and shall be retained by the Zoning Administrator as a public record. An application for a zoning certificate shall also have affixed to it the certificate of an architect or registered professional engineer licensed by the state that the building or structure and the proposed use thereof comply with all of the provisions of this chapter.
