Application for a permit required by § 150.012 shall be in writing in the form prescribed by the village. Such application shall contain such information and drawings as shall be required by the village for a proper understanding of the proposed work. Each application for permit for the construction of a public pool shall be accompanied by an approval permit for the construction issued by the County Health Department and reviewed by Illinois Environmental Protection Agency and North Shore Sanitation District. Applications for permits shall be accompanied by plans and specifications in duplicate and in sufficient detail shall show the following:
(A) Plot plan elevation with dimensions drawn to scale.
(B) Pool dimensions, depth, and volume in gallons.
(C) Type and size of filter systems, filtration and backwash capacity.
(D) Pool piping layout with all pipe sized, and valves shown and types of materials to be used.
(E) The rated capacity and head of filtration and backwash flow of the pool pump in gallons per minute with the size and type of motor.
(F) Location and type of waste disposal system.
