(A) The application for a license required by § 113.25 shall contain the names and addresses of all persons having a financial interest in such an establishment, the type of business proposed, and the address of the place proposed to conduct the business of operating a tanning salon.

(B) The annual fee for the license shall be $25 for each establishment, and in addition $5 for each attendant of such establishment renewable yearly. Licenses shall be issued concurrently with the fiscal year of the village.
