(a) No person, firm or corporation shall construct, alter, remove or demolish or commence the construction, alteration, removal or demolition of any building or structure, or install any equipment for the operation of a building or structure without first filing with the Code Enforcement Officer, an application in writing and obtaining a formal permit as required by the Zoning Ordinance. (b) No person, firm or corporation either as a contractor or subcontractor shall perform any of the services set forth in subsection (a) hereof without first obtaining a permit from the Code Enforcement Officer to perform said work within the Municipality, which permit shall be issued by the Code Enforcement Officer upon the payment of a fee of seventy-five dollars ($75.00) per annum, and in addition thereto shall have the following requirements.

(1) Proof of compliance with the Worker’s Compensation Laws of the State of Ohio; and
(2) Proof of liability insurance either in the State of Ohio or if a nonresident person, firm or corporation, compliance within the home state. Insurance shall be in a minimum amount of one hundred thousand dollars ($100,000) per occurrence.
(c) The failure of any person, firm or corporation to meet any of the foregoing requirements, shall cause the Code Enforcement Officer to refuse to issue a permit for the performance of any work by said contractor or subcontractor within the Municipality.
