The application for a license shall contain the following information:
(a) Name and address, age and citizenship, if a corporation, the officers thereof: no license shall be issued to any applicant unless he shall be twenty-one years of age and a citizen of the United States;
(b) Prior convictions, other than traffic violations;
(c) Description of machine or machines to be covered: a distributor shall within five days report any new machines to the Mayor’s Office;
(d) The type of business conducted at the place where the machine is operated;
(e) Adequate proof that personal property taxes have been paid on the equipment.
