Any person desiring a license for any motor vehicle shall file an application with the Village Manager, upon such forms as the Village Manager shall prescribe, setting forth the name and address of the applicant, a description of the motor vehicle for which the license is desired, and the place where such motor vehicle is to be kept when not in use, together with such information as may be required on such forms.
