Whenever a revocation of a license to operate a food service establishment or retail food store has become final, the holder of such revoked license may apply for a new license to operate as provided in Section 8.12.060. In the case that revocation was for violations of a nature other than physical structure or equipment, a written plan of action may be required as part of the new application process.
