A. Applicability. The application requirements and procedures established in this section shall apply to all taxicab operator's license applications, whether such application is for a new license or for the renewal of an existing license.
B. Form of application.
1. Application for a taxicab operator's license shall be made in writing to the Chief of Police on a form provided by the Village.
2. A separate application shall be required for each vehicle driver.
C. Contents of application. The application shall contain the applicant information and documentation required in this chapter and such other and additional information and documentation as the Chief of Police may deem necessary to determine the qualifications and eligibility of an applicant for a license to engage in the taxicab business in the Village.
D. Applicant information. The application shall contain at least the following information:
1. the applicant's name, home address and telephone number;
2. the business name, address and telephone number of the applicant's taxicab business, if different from the personal information provided pursuant to the foregoing paragraph;
3. the name, address and telephone number of any person or business that will perform dispatch services for the applicant's taxicab business;
4. the applicant's driver's license number;
5. the applicant's employment history, including the name of the current employer and length of service with the current employer;
6. the names and contact information of at least two responsible citizens as references;
7. the applicant's record of criminal convictions, if any; and
8. the applicant's driving record, including traffic violations and the driver's experience in transportation of passengers for hire, if any.
E. Determination of eligibility. No license shall be issued to any applicant unless the Chief of Police has determined that the applicant has demonstrated compliance with the following requirements:
1. the applicant has submitted a complete application;
2. the applicant is at least 18 years old;
3. the applicant has a valid public passenger driver's license;
4. the applicant meets the bond and insurance requirements established in this chapter;
5. the applicant meets the vehicle safety and inspection requirements established in this chapter, which shall include but not be limited to providing proof that the vehicle proposed to be used in the applicant's taxicab business has passed a safety lane inspection within the 6 months preceding the date of the application;
6. the applicant has the requisite character, physical faculties and ability to drive a motor vehicle on the streets of the Village in a safe and efficient manner;
7. the applicant possesses other qualities of general fitness for driving a taxicab; and
8. the applicant has paid the applicable license fee.
