§ 111.15 LICENSING AND REGISTRATION; APPLICATION PROCEDURE.
   (A)   Contractors shall have passed a test in their respective and specific trade from any Lake County, Indiana, governmental unit, as proof of licensure, before seeking registration with the town, unless no test for that trade is available in Lake County. Upon passing a test in their respective and specific trade, unless no test exists for that trade, as identified in this chapter, a contractor and/or tradesman may file an application for registration with the town, on forms provided by the Administrator, in the name of the business entity of which the contractor conducts business. The application shall be signed by the individual, general partner, or corporate officer who has legal responsibility for the business entity and any employees thereof. In situations where a business entity may have more than one legally responsible person or corporate officer, that information should also be provided. The application shall also include at least one contact person for normal business needs and one emergency contact person available outside normal business hours.
   (B)    The Administrator may require adequate proof of the type of business entity and personal identification documents. The Administrator may take time to make reasonable inquiries to verify information provided on the application.
   (C)   The application must include a list of all the activities and professions, as listed in § 111.05 above, that the contractor and/or tradesman conducts within the town. The listing must include an individual responsible and a contact person for each activity and profession.
   (D)   A contractor must have the registration application approved by the Administrator or staff before it is valid. The Administrator may approve the registration either as active status or as inactive status.
