Applications for such licenses shall be made to the Local Liquor Control Commissioner, in writing, under oath, and signed by the applicant, in accordance with the directions set forth in the application. License personal interest forms shall be completed in writing, under oath, for each person signing the application and each manager employed or to be employed at the licensed premises. Complete application for any license renewal (i.e., liquor license application and all liquor license personal interest forms, and all attachments thereto) shall be submitted at least thirty (30) days prior to the beginning of any license year. Failure to do so may result in a delay in the renewal license being issued. The form of liquor license application and liquor license personal interest form shall be as follows:
