12-1-5: REQUIRED WRITTEN NOTICE:
Throughout the management of this procedure, it is explicitly understood that prior to the appeal of an employee to a higher level of supervision, including, when appropriate, the Village Administrator, all intermediate supervisory levels and the Supervisor to which the appeal is being made shall be notified of the request in writing. This written document shall include the name of the employee, the Supervisor to which the appeal is being made, a short description of the problem, a statement of any proposed remedies, and the signature of all intermediate supervisory levels acknowledging awareness of the appeal.
