The application for a permit shall be on a form approved by the Director of Public Service and shall contain the following information:
(a) The location of the proposed sidewalk, apron or curb.
(b) The name and address of the owner of the parcel, the permanent parcel number, and the name of the contractor.
(c) A drawing showing the location of the proposed sidewalk, apron, curb or driveway which driveway shall show the distance from the contiguous property and from the paved portion of any paved public right-of-way.
