The issuance of the permit shall be conditioned on the following:
(a) The work shall be done only in the location authorized by the permit and approved by Council.
(b) Any street repair or improvement shall be done by the permittee at its cost.
(c) If necessary, any existing utility or other service lines that require relocation, movement, or replacement shall be done by the permittee and at its cost. Any such work shall be approved by the owner or user of said utilities or lines.
(d) The method of controlling traffic and accommodating the uses of the right-of-way shall be approved by the City of Willoughby Police Department.
(e) The plans and specifications shall be approved by the City Engineer.
(f) The public way and any other grounds and facilities damaged in the area in which the work is to be performed shall be returned to the condition existing before the commencement of the work at permittee's cost and said restoration shall be approved by the City Engineer. In the event of the failure to repair such public way or area, permittee shall pay to the City the cost and expense expended by the City in repairing such damages.
