No Certificate of Public Convenience and Necessity shall be issued, nor shall any taxicab be operated upon the streets of the City, until such taxicab operator has filed with the Mayor a statement showing:
(a) The make of the motor car to be used;
(b) The State license number of each car;
(c) The vehicle identification number of each car;
(d) The name of the owner of the car;
(e) The factory number of each car;
(f) The seating capacity as originally purchased and, if changed or reconstructed, the seating capacity in its changed form; and
(g) The name of the drivers to be in charge of the operation of the cars and their places of residence.
