No person shall drive a school bus in this City unless he is licensed to do so. An applicant for a driver's license shall:
(a) Be twenty-one years of age or over;
(b) Be of sound physique, with good eyesight and not subject to epilepsy, vertigo, heart trouble or any other infirmity of body or mind which might render him unfit for the safe operation of a school bus;
(c) Be able to read, write and speak the English language;
(d) Be clean in dress and person, of good moral character and not addicted to the use of intoxicating liquors or narcotic drugs;
(e) Be the holder of a current Ohio driver's or chauffeur's license;
(f) Produce, on a form to be provided by the Director of Public Safety, affidavits of his good character from two reputable citizens of the City or of Lake or Cuyahoga Counties, who have known him personally, and a further testimonial on a form provided for that purpose, from his last employer, unless, in the opinion of the Director, sufficient reason is given for its omission; and
(g) Fill out, on a form to be provided by the Director, a statement giving his full name, residence, experience in driving motor vehicles, places of residence for five years previous to moving to his present address, age, color, height, color of eyes and hair, place of birth, length of time he has resided in Lake or Cuyahoga Counties, whether a citizen of the United States, place of previous employment, whether married or single, whether he has been summoned to court, whether he has previously been licensed as a driver or chauffeur, and if so, whether any such license has ever been revoked and for what cause, which statement shall be signed and sworn to by the applicant and filed with the Director as a permanent record. The Director is hereby authorized to establish such additional rules and regulations covering the issuance of drivers' licenses, not inconsistent herewith, as may be necessary and reasonable.
