Any person desiring to open or maintain such a room or place shall make written application to the Finance Director for a license for such purpose. Such application shall state the name and residence address, including street and number, of every person interested in the opening or maintaining of such room or place, and if a corporation is so interested, the names and residence addresses of the officers thereof. The application shall also state the premises where such room or place is to be opened or maintained, the name and resident address of the manager or person in charge thereof, whether any of the persons required to be named in the application have been engaged in the operation of such a room or place within five years next preceding the application, and when, where and how long they have been so engaged.
