(a) Whenever a vacancy exists for an original appointment, the appointing authority shall ask the Personnel Director to certify the names of persons eligible for that appointment. The three persons with the highest composite ranks on the original appointment eligibility list shall be certified.
(b) The appointing authorities shall appoint one of those certified, notifying the Director, so that the person appointed may be placed upon the payroll and the persons not appointed may be restored to the eligibility list.
(c) If anyone on the eligibility list is certified and appointed for an original appointment, but refuses the same, his or her name shall remain on the list. However, in the event of a second refusal, his or her name shall be stricken from the list.
