1112.03 APPLICATION; REGISTRATION FEE; RENEWAL FEE.
   (a)   Application for a certificate of registration shall be upon a form provided by the Building Commissioner and shall be accompanied by a registration fee of one hundred dollars ($100.00). The application form shall include space for the true name, current address, and telephone number of the contractor applying, and designate the form of business organization, whether corporation, partnership, individual or other organization. When the applicant is other than an individual, he shall state the name, address and telephone number of an individual charged with the management of the work of the business organization to be performed within the City. The application shall contain such other information as the Building Commissioner deems pertinent.
   (b)   Upon the filing of the application and bond, if they are in conformity with the provisions of this chapter, the Building Commissioner is hereby directed to issue to applicants who have given satisfactory proof of the knowledge of the essentials of the business in which they intend to engage, a certificate of registration upon payment of the above-stated fee.
   (c)   The certificate of registration shall be for the term of one calendar year, commencing on January 1 and ending on December 31, and may be renewed annually upon the payment of a renewal fee of one hundred dollars ($100.00).
