Every licensee shall furnish to the Director of Public Safety not later than the third day of each month a full and complete statement of claims filed and judgements rendered against the licensee arising out of the operation of taxicabs or vehicles, which statement shall be in such form as the Director shall prescribe. Failure to comply with the requirements of this section shall be subject to the penalty provided in Section 763.99 and each days failure or neglect shall constitute a separate offense.
