Prior to the issuance of each permit, the applicant shall furnish the make, model, and serial number of the vehicle or equipment to be used and the name, address and telephone number of the owner and operator of such vehicle or equipment. A permit shall be required for each vehicle or equipment used to plow or remove snow. Each applicant shall pay the sum of ten dollars ($10.00) for each permit for each vehicle or equipment. Each permit shall remain in effect from October 15 to April 15 of each year.
