The application shall be made on forms required by the city and provide at least the following information, where applicable. Additional information may be required on the permit application forms.
A. The proposed elevation in relation to mean sea level, of the lowest floor (including basement) of all residential and nonresidential structures whether new or substantially improved to be located in zones A, A1-A30, AE, and AH, if base flood elevations data are available.
B. The proposed elevation in relation to mean sea level, of the lowest floor (including basement) and the elevation of the highest adjacent grade of all residential and nonresidential structures whether new or substantially improved to be located in Zone AO.
C. The proposed elevation in relation to mean sea level, to which any new or substantially improved nonresidential structure will be floodproofed.
D. A certificate from a licensed professional engineer or architect in the State for any utility floodproofing will meet the criteria in Section 10-1-29 of this Chapter.
E. A certificate from a licensed professional engineer or architect in the State for any nonresidential floodproofed structures will meet the criteria in Section 10-1-27 of this Chapter.
F. A description of the extent to which any watercourse will be altered or relocated as a result of the proposed development. Computations by a licensed professional engineer in the State must be submitted that demonstrates the altered or relocated segment will provide equal or greater conveyance than the original stream segment. The applicant must submit any maps, computations or other material required by the Federal Emergency Management Agency (FEMA) to revise the documents enumerated in Section 10-1-5 of this Chapter, when notified by the Local Flood Plain Administrator, and must also provide assurances that the conveyance capacity of the altered or relocated stream segment will be maintained.
G. In certain circumstances the Local Flood Plain Administrator will require a technical analysis, by a licensed professional engineer in the State, showing the proposed development located in the special flood hazard area will not cause physical damage to any other property.
H. When there is no base flood elevation data available for Zone A from any source, the base flood elevation data will be provided by the permit applicant for all proposed development of subdivisions, manufactured home and recreational vehicle parks in the special flood hazard areas, for all developments of fifty (50) lots to five (5) acres, whichever is less.
