(a) Pursuant to Ohio R.C. 9.15 the Village will allow and shall pay burial expenses of indigent persons found to be a legal resident of the Municipality at the time of death. Expenses for burial are limited to a maximum amount up to seven hundred and fifty dollars ($750.00). In addition, the Village shall provide a stone or concrete marker with the person’s name, age, if known, and date of death inscribed. The Village shall pay an amount up to two hundred and fifty dollars ($250.00) for the cost of the marker.
(b) Burial shall occur in a lot and cemetery as approved by the Village. If another burial location is requested, the purchase price for the burial lot shall not be an allowed expense and shall not be paid by the Village.
(c) All persons seeking reimbursement from the Village for indigent burial expenses shall provide the following to the Fiscal Officer:
(1) Information establishing indigency on a form to be furnished by the Fiscal Officer.
(2) Copy of death certificate.
(3) Copy of bills to be paid.
Payment shall not be made without the above documentation.
(d) Should it be ascertained after any payment by the Village that such payment was made for a decedent whom the Village did not have the responsibility to bury pursuant to Ohio R.C. 9.15, the Village shall be entitled to any benefits payable on behalf of such decedent by any person or agency who had the responsibility to pay burial benefits, up to an amount totaling the payment made by the Village.
