709.05 ADDITIONAL INFORMATION.
   (a)   Application for a canvasser’s or peddler’s license shall also contain the following:
      (1)   The place where the goods or property proposed to be sold, or orders taken for the sale thereof, are manufactured or produced and where the goods or products are located at the time the application is filed, and the proposed method of delivery, if applicable;
      (2)   If it is asserted that any warranties or guarantees are made or offered in conjunction with the sale, a copy of such warranty or guaranty must be attached to the application, together with a specific address for exercising the warranty or guaranty rights;
      (3)   Such other information as the Mayor may require to carry out the purposes set forth in Section 709.01.
   (b)   The application for a solicitor’s license shall also contain the following:
      (1)   Whether the applicant claims that he represents a tax exempt organization, and if so, the basis for such claim, including, copies of any IRS exemption letter;
      (2)   A detailed description of the purpose of the solicitation including the specific use that will be made of the funds collected and the specific name, address and telephone number of the organization on behalf of which such funds are being collected;
      (3)   Such other information as the Mayor may require to carry out the purposes set forth in Section 709.01.
