Applicants for a permit and license under this article shall file with the City Clerk a sworn application in writing in duplicate on a form to be furnished by the City Clerk which shall give the following information:
(a) Name and description of applicant;
(b) Permanent home address and local address;
(c) Description of the nature of the business and goods to be sold;
(d) Name and address of any employer with evidence establishing the exact relationship;
(e) The length of time for which the right to do business is desired;
(f) The place where the goods or property to be sold or orders taken therefor are manufactured or produced and the proposed method of delivery;
(g) Names and addresses of two persons who can certify as to the applicant's character and business respectability;
(h) Statement as to any conviction of a felony or misdemeanor of the applicant;
(i) Such other information as the City Clerk may require.
At the time of the filing of the application a fee of one dollar ($1.00) shall be paid to the City Clerk.
