134.01 SERVICE CHARGES.
   (a)   The service for the West Liberty EMS will be charging/billing for each medical run within the West Liberty Area. Basic Life Support charges will be as follows:
      (1)   Basic Life Support (BLS) Non-Emergency: Four hundred dollars ($400.00).
      (2)   Basic Life Support (BLS) Emergency: Four hundred dollars ($400.00).
      (3)   Basic Life Support (BLS) Loaded Mileage: Ten dollars ($10.00).
   (b)   The service for the West Liberty EMS will be charging/billing for each medical run within the West Liberty Area. Advanced Life Support charges will be as follows:
      (1)   Advanced Life Support (ALS) Level 1: Four hundred fifty dollars ($450.00).
The provision of an assessment by an ALS provider or supplier or the provision of one or more ALS interventions. An ALS provider/supplier is defined as a provider trained to the level of the EMS-Intermediate or Paramedic. An ALS intervention is defined as procedure beyond the scope of an EMS-Basic.
      (2)   Advanced Life Support (ALS) Level 1 Emergency: Five hundred dollars ($500.00). As above, in the context of an emergency response. An emergency response is one that, at the time the ambulance is called, is provided after the sudden onset of a medical condition manifesting itself by acute symptoms of sufficient severity such that the absence of immediate medical attention could reasonably be expected to result in placing the beneficiary’s health in serious jeopardy; in impairment to bodily functions; or in serious dysfunction to any bodily organ or part.
      (3)   Advanced Life Support (ALS) Level 2: Six hundred fifty dollars ($650.00).
The administration of three or more different medications and/or the provision of at least one of the following ALS procedures: Manual defibrillation/cardioversion, endotrachceal intubation, central venous line, cardiac pacing, chest decompression, surgical airway, intraosseous line.
      (4)   Advanced Life Support (ALS) Loaded Mileage: Ten dollars ($10.00).
   (c)   The service for the West Liberty EMS will be charging/billing for each medical run within the West Liberty in the amount of seventy-five dollars ($75.00) if not transported.
   (d)   The service for the West Liberty EMS will be charging/billing five hundred dollars ($500.00) for helicopter prep within the West Liberty Area.
