Whereas, this Council believes that the health, safety and the welfare of the citizenry should be of the utmost concern to those employees who provide the varied Village services; and whereas, certain information regarding any employee’s secondary employment is vital not only for determining the liability of the Village in such matters as unemployment compensation, Worker’s Compensation, and the like, but also to insure that each employee is able to perform his/her job so that the citizenry receives the service it demands. Therefore, any employee who engages in employment in addition to his regular Village employment shall immediately contact the Mayor, and furnish in writing the following information:
(a) Employee’s name and Village position.
(b) Additional employer’s name, address and phone number.
(c) Nature of additional employment.
(d) Normal hours of employment at additional position.
(e) Whether the additional employer has been advised that said employee is required to report for Village duty in the event of an emergency.
