Application for a license or permit shall be made in writing to the Village Clerk, in the absence of a provision to the contrary. Such application shall be made on a form provided by the Clerk for that purpose. The applicant shall state, in all cases, the name and business address of the applicant, also, if by a firm, the names and addresses of each general partner, and if by a corporation, the names and addresses of the principal officers. In addition, the application shall set forth a description of the business or thing for which license or permit is sought, the location or proposed location of such business, the number of employees and of vehicles employed, if any, and such other information as may be required by this Code in regard to the particular application. The application shall be signed by the applicant. Forms for all licenses and permits, and applications therefor, shall be prepared and kept on file by the Village Clerk under the general direction of the Board of Trustees.
