§ 92.02 APPLICATION AND PLAN.
   (A)   The application shall give the location and dimensions of the proposed opening, the purpose for which the opening is to be made, the kind of pavement or surface to be opened, and the approximate time when the cut will be made. Permits will be issued only for cuts to be made within one week of the date of issue, unless special arrangements for a longer period of time have been approved. The period of time allowed for closing the opening will be governed by its size and nature. It will also be required that all applications for mains, conduits, manholes, and other major subsurface structures be accompanied by a plan and typical cross-section, which will show as accurately as possible the location of the existing underground structures, the location of the proposed structure, a cross-section of the existing paving through which the cut is to be made and the location of all construction joints and dummy joints in the existing paving. These plans shall be submitted in duplicate. The issuance of a permit will be based on the approved plans. No deviation from the approved plans will be permitted until such deviation has been approved.
   (B)   When changes in plans have been authorized, “as built” plans showing all details as actually constructed shall be filed, upon completion of the work.
   (C)   The building inspector shall require the applicant to furnish satisfactory evidence that he is keeping in full force and effect a liability insurance policy, furnishing to himself and the city protection against claims arising out of the performance of the work covered under the permit applied for, or in any way connected therewith, in an amount of $25,000 for any one person injured in any accident, and with a total liability of $50,000 for all persons injured in any one accident, and the amount of $10,000 for each accident with an aggregate of $25,000 as compensation for damage caused to the property of any person other than that belonging to the applicant.
