Applications for the license required by § 111.02 shall be made to the Mayor or the Service-Safety Director upon forms furnished, and the applicant shall swear to the truth of the contents of the application and sign the application. In addition to the license application, the applicant shall furnish to the Service-Safety Director his or her permanent address.
