(a) The initial application shall be on a form designated by and submitted to the Village Manager. The application shall include, among other things, identification of the applicant, a description of the proposed location of the installation, and, if the applicant is a "utility" recognized by the State of Ohio, shall include a copy of such certification.
(b) The permit fee shall be the lesser of two hundred fifty dollars ($250.00) or the amount currently charged by the Village for a building permit for commercial development.
