757.04 PERMIT PROCEDURES AND FEE.
   (a)   Each person, firm or corporation desiring to own, operate or manage a game room in the Municipality shall first make application for same on forms provided by the Municipality and shall furnish such data as may be required which shall include the full name and address of the applicant, and of the manager or other person in charge, the location for which the permit is sought, the type and quantity of pinball machines, electronic games or similar amusement devices to be installed or exhibited and such other information as may be deemed necessary for the enforcement of this chapter.
 
   (b)   An annual fee of five hundred dollars ($500.00) shall accompany each application.
 
   (c)   A permit shall be valid for one year from date of its issue and may be renewed in the manner and for the fee set forth above so long as all other regulations are met.
 
   (d)   No person, firm or corporation shall fail to display a proper permit upon proper request therefor.
