(A) Any person desiring a permit to keep, store or transport explosives within the city, as required in § 513.01, shall make written application to the Fire Chief therefor.
(B) Name, address, and daytime phone number of the applicant. If the applicant is a corporation or business entity, only the chief executive officer or equivalent of that corporation ot business entity may sign the application. The application shall contain, at a minimum, the following:
(1) Name, address, and phone number of the applicant;
(2) Name, address, experience, qualifications, and daytime phone number of blasting supervisor;
(3) Copy of blasting supervisor's qualifications and licensure;
(4) Blasting plan which meets the specifications set forth in § 513.04;
(5) Certificates of general liability and property damage insurance issued by a carrier authorized by the State of West Virginia Insurance Commissioner to do business in West Virginia for, at a minimum, $2,000,000 combined single limit per occurrence. The city shall be named as an insured on each policy.
(C) Upon the filing of such application, the Fire Chief may examine the applicant and the magazine, if the explosives are to be stored, and the use to which such explosives are to be put. If, upon such examination, the Fire Chief shall find that the things required as set forth in the application are true and in proper form, he or she may, in conformity with this article, issue and deliver to such applicant a permit, without charge, for the period mentioned in such application.
