§ 35.50 AGREEMENT FOR AUTHORIZED USE.

   (A)   Village purchasing card policy.

   (B)   Card user agreement for authorized use of purchase card.

      I,                                               , have read and do understand the village’s purchasing card policy.

      I further agree to abide by the terms and conditions in that policy and understand that my use of the card is subject to revocation if I fail to act in accordance with the policy.

      I also understand that if I engage in fraudulent misuse of the village’s purchasing card, I will be held responsible for repayment of these funds and it may result in employment termination.
