909.04  MINIMUM REQUIREMENTS.
   All openings shall be made and restored with the approval of the Street Superintendent, and deposits released after an approval of release by the Street Superintendent. All excavations shall be made under the following minimum requirements:
   (a)   Dirt excavations shall be restored with dirt of the same quality, tamped and secured in place to restore the subsurface and surface to the former condition, and reseeded if grass is on the surface.
   (b)   Excavations under the pavement shall be filled with screenings and tapped into place below the pavement surface by power tapping or water sucking, satisfactory to the Street Superintendent, and all pavement surfaces restored as prior to excavation on the level of the existing pavement.
   (c)   Barriers shall be maintained at all times and guard lights during darkness.
   (d)   Appropriate warning signs shall be erected and maintained.
   (e)   All liabilities for injury to persons or property by reason of excavations shall be assumed by the permittee. The deposit of a liability insurance certificate may be required if ordered by the Municipal Administrator prior to issuance of a permit.
