909.03  APPLICATION AND CASH DEPOSIT.
   (a)   Each permit for making an opening shall be confined to a single project and shall be issued only by the Public Works Director or his or her designee.  Application shall be made on a form prescribed by the Public Works Director, giving the exact location of the proposed opening, the kind of paving, the area and depth to be excavated, and such other facts as may be required.  The permit  shall be issued only after a cash deposit or a surety bond of a surety or bonding company approved by the Solicitor sufficient to cover the cost of restoration has been posted with the Public Works Director, conditioned on prompt and satisfactory refilling of excavation and restoration of all surfaces disturbed.
   (b)   The cash deposit or bond shall be in an amount as determined from time to time by Council.
   (c)   Additional amounts may be required by the Public Works Director on determination of circumstances justifying an increased deposit.
